Step
1
— Click here to print out a
New Account Information Form.
Fill it out and bring it to one of our convenient locations. You
will need to bring a form of identification, such as a valid
driver's license.

2
— Stop using your old checking account. Keep your old checking
account open until all your direct deposits and automatic payments
take effect and until ALL outstanding checks clear.

Automatic Payment Checklist.
List all your automatic payments that you will need to change or set
up to be deducted from your new account. Print out each of the forms
below as applicable. Fill out the forms and mail them to each
company that you have automatic payments or direct deposits with or
bring the forms to any of our convenient locations and let us help
you.
Direct Deposit/Automatic Payment
Enrollment Authorization Form
- This is a dual purpose form that allows you to have your direct
deposits or automatic payments redirected to your new First National
Bank checking account.
1199A Direct Deposit Sign Up Form
- Military and government employees need to use this form for direct
deposits. The fastest way to change direct deposit of social

Please Close My Account
- After all your checks have cleared your old bank and all direct
deposits and automatic payments have been redirected to your new
account at First National Bank, complete this form and mail to your
old bank to close your account.
